liability waiver+ assumption
of risk

I understand that participation in athletic training activities provided by Athlete Lab Hawaiʻi LLC involves inherent risks, including but not limited to:

• Sprains, strains, and muscle injuries

• Fractures or broken bones

• Concussions or head injuries

• Heat exhaustion, dehydration, or illness

• Collisions with other participants or equipment

• Slips, trips, and falls

• Aggravation of pre-existing injuries or conditions

I understand that these risks exist even when proper supervision, instruction, and safety precautions are in place.

I confirm that participation in all training activities is voluntary, and I (or my child) am physically able to participate. I choose to participate despite the risks described above.

I voluntarily assume full responsibility for any risks of injury, illness, or death that may occur during participation in Athlete Lab Hawaiʻi LLC activities, whether caused by negligence or otherwise, to the fullest extent permitted by law.

I hereby release, waive, discharge, and hold harmless Athlete Lab Hawaiʻi LLC, its owners, coaches, employees, contractors, and affiliates from any and all claims, liabilities, damages, or causes of action arising from participation in training activities.

This includes any injury or loss caused in whole or in part by negligence.

In the event of an emergency, I authorize Athlete Lab Hawaiʻi LLC staff to seek emergency medical care for the participant.

I understand that I am responsible for all medical expenses incurred.

I certify that the participant:

• Has no medical conditions that would prevent safe participation, OR

• Has disclosed all relevant conditions in my child’s registration form

I understand that Athlete Lab Hawaiʻi LLC may remove a participant from training without refund if the athlete:

• Poses a safety risk

• Disrupts training sessions

• Fails to follow coach instructions

• Displays inappropriate or unsafe behavior

I agree to indemnify and hold harmless Athlete Lab Hawaiʻi LLC from any claims arising out of participation in Activities.

This agreement shall be governed by the laws of the State of Hawaiʻi.

I certify that I am the parent or legal guardian of the registered athlete.

I have read, understand, and voluntarily agree to the Athlete Lab Hawaiʻi Liability Waiver & Assumption of Risk. I understand that checking this box constitutes my electronic signature and acceptance of all terms and conditions outlined above.
