media release

I understand that Athlete Lab Hawaiʻi LLC may photograph, video record, or otherwise capture my child during training sessions, events, or activities.

I grant Athlete Lab Hawaiʻi LLC permission to use photographs, video recordings, audio recordings, or other media containing my child's likeness for lawful business purposes, including but not limited to:

• Social media platforms

• Website content

• Promotional videos

• Marketing materials

• Printed advertisements

• Educational content

• News or media publications

• Future advertising and promotional campaigns

I understand that no compensation or royalties will be provided for the use of these images or recordings.

I understand that Athlete Lab Hawaiʻi LLC owns all rights to any photographs, videos, or other media created during its programs.

Athlete Lab Hawaiʻi LLC will never intentionally publish:

• Home address

• Phone number

• Email address

• Medical information

• School information

• Any other confidential personal information

Athlete names may occasionally be used in social media posts or promotional materials unless a parent or guardian requests otherwise in writing.

Athlete Lab Hawaiʻi LLC respectfully accommodates families who prefer their child not appear in photographs or videos.

Please email: info@athletelabhawaii.com torequest that your child NOT be photographed or recorded during Athlete Lab Hawaiʻi LLC activities.

I understand Athlete Lab Hawaiʻi LLC will make reasonable efforts to honor this request but cannot guarantee my child will never appear incidentally in group photographs or videos taken during training sessions or events.

I release and discharge Athlete Lab Hawaiʻi LLC, its owners, coaches, employees, contractors, volunteers, and representatives from any claims arising from the lawful use of photographs, videos, or recordings authorized under this agreement.

This Media Release shall remain in effect for as long as my child participates in Athlete Lab Hawaiʻi LLC programs unless revoked in writing.

A written revocation will apply only to future media captured after Athlete Lab Hawaiʻi LLC receives the request.

I certify that I am the parent or legal guardian of the registered athlete.

I have read, understand, and agree to the Athlete Lab Hawaiʻi Membership Agreement. I understand that checking this box constitutes my electronic signature and acceptance of all terms and conditions outlined above.
